Prepare New Hdd after configuring disk [FAIL]

hi everyone,

i'm using AutoInstall script to prepare my new hdd, everything is fine,
but after the restart server, i cannot see my share volume..

this is my boot.log! where is the problem? please help.

holy shit.. i fixed..

I solved by using an alternative procedure for preparation of sda5.
